A 24-hour notice is required for cancelation of an appointment, or you will be charged in full for the appointment. These calls must be received Monday – Friday business hours 9 AM – 6 PM. Payment is due before your next appointment.
Appointment times are as scheduled and cannot extend beyond the stated time to accommodate late arrivals. Please be on time to your appointment.
Massage/bodywork is not appropriate care for infectious or contagious illness. Please cancel your appointment as soon as you are aware of an infectious or contagious condition. If it is within the 24-hour notice period, the cancelation fee may be waived.
You assume financial responsibility for all services regardless of insurance reimbursement. First time appointments made within the 24 Hour Cancelation period will need advance payment to reserve appointment time.
Release of Medical Records
Your signature below authorizes the release of all of your medical records on file in this office to the following: your attorney, the healthcare providers attending to this condition, and the insurance case managers. Medical records will not be edited unless otherwise stated in an exclusive release of medical records signed through your attorney.
Right to Terminate Session
Each party has the right to terminate a session at any time for inappropriate behavior, intoxication, infectious condition (i.e. flu, cold, etc), sexual advances. If the client expresses interest in a sexual massage, the session will be terminated immediately and payment for full service will be rendered in full.
